Primary Care Associates accepts most health insurance. HMO/PPO list You will pay your specified co-pay amount for each office visit or covered procedure. Our insurance department will file your insurance claim with your health insurance carrier.

Please bring your insurance card with you for your first visit. We will make a copy of your card for your file. We may also ask you additional questions about your insurance if we need more information. Please bring your card with you whenever you visit PCA.

Coverage may vary from one insurance carrier to another. In most cases, laboratory services, bone densitometry testing, and physical therapy rehabilitation are covered when ordered by your physician.
